Why?

To make your report complete, be sure to include the: • The Who: Who were the individuals involved? Who responded? Who was notified? Were you able to collect information or description of those involved? • The What: What actually happened? What was the response? What did you observe or hear? Explain the chronological events that took place and be as descriptive as possible. • The When: When did the incident occur? When was it reported? Try to always include the time, and if you're unsure, use a window of time and avoid guessing. • The Where: Where did the incident occur? Include details like the address, location, scenery, and any other pertinent information. • The Why: Why did the incident occur? Why did security respond? This part can be challenging to answer, so never speculate and only report what you know happened. • The How: How did the incident occur? How was it reported? How did you hear about it? How was the situation resolved? Only include facts and details that you are sure of.
