Why?

Yes, you should be clear and precise when giving directions, but it's okay to repeat yourself if the person asks you to do so! Hearing loss is different for everyone. Sometimes people can hear sounds, but they can't make out speech, or they may have some residual hearing and can understand what you're saying if you speak louder.
